Howdy Folks! Wanted to make this thread for those folks that had their Citi Strata Elite account locked and Citi requesting a 4506-C Tax Form to remove the hold. I hope this space can be a place for folks to put updates as im assuming forms should be hitting mail boxes soon.

    For me, I didn’t wait for the form to be mailed to me and printed out the sheet that I posted in an earlier thread with the help of an agent. Expressed Mailed it on Friday 9.05.25 and it arrived in their mail box on 9.08.25. I called yesterday evening on 9.09.25 to see if they had it in the system, they did apparently, but they did not submit it in until I asked if it was going to be submitted. They also double checked if all the info was correct and they confirmed twice it was. So now that request went to the IRS system im assuming (I did call again this morning 9.10.25 and they did forward the request.) Now i will follow up with them Friday 9.12.25 and Tuesday 9.16.25 to see any updates.

    My recommendation is to call them when you think the mail arrived to them to ensure they actually send it in.

    So my questions are:
    1. Did you receive the form, how (printed it or snail mail)? How did you mail it back?
    2. When your account was locked and what SUB did you use?
    3. Did you call to confirm it was submitted? (Basically asking if you asked them to do their job)
    4. Any other updates you want to share with the class as time goes on.

    1. Had to call before seeing 4506-C showed in online account.

      Keep an eye on the form. It is not filled out fully. Mine was missing the check box (6a) for Return Transcript and it was missing the date of return (8).

      The IVES listed is 300501. I saw in prior posts/DPs that number was 30005 or some other variation.

      Good luck everyone. No clue how long they are going to take to resolve this. Btwn the time it takes to send this to them, for them to get the transcript, and then review, I’m missing out on big charges I need to make and now can’t apply it to this card.

      I sent the form in priority mail — they should get it later this week. I’ve already received the SUB and am debating whether I should use the points to buy gift cards before Citi receives the form, just in case they find something wrong with it (it is a bit lower than reported income due to expected increase in income). Thoughts?

    3. 1. For the first time, I can actually access my account online again, by just clicking through the fraud alert at login. Has this happened to anyone else? Will try out whether my Strata Premier and/or Strata Elite work now again, they didn’t last week. **I)** Did this happen to anyone else? Could you use the card? **II)** I’m kind of considering transferring out all the points (already reached SUB), in case they keep screwing this up. Seems a bit of a rash move, but could be a calculated move to avoid the risk of complete shutdown – since they’re able to do basically what they want.
      2. Kind of outraged over CSRs providing what seems like faulty information for how to fill in the form. I am traveling, so called in asking what I needed to do. Based on others’ comments, it seems we need to fill in 5a, 5d, 6 – which the CSR told me I could leave all blank. **III)** Is it possible they will type this in themselves on my first form I sent? (My understanding is it goes through Citi then to the IRS) Or should I 100% send a new one? **IV)** Did you leave 5b all unfilled? Or was it prefilled the way it arrived in the mail?
