Morning all. ETS a few months ago and tried to log on to MyPay to access my W2 for taxes and I got hit with this error message.
Your ACCOUNT has been disabled and you are prohibited from accessing this system. For procedures on how to activate your Account, contact a customer service representative by calling 1-888-DFAS411 or 1-888-332-7411 or 317-212-0550.
Friends of mine who ETS'd in the same month as me are able to logon still. Called DFAS and they want me to fill out some form, have it notarized, and send it back in to unlock my account. Never had any issues accessing before this.
Just curious if anyone else ever had this issue?

Happened to me after a password of mine got leaked that was also my DS login. Call the 317 number. They will verify your info and then email you the form. You take a picture of an ID (DL or CAC), then digitally sign the form and email it back. My account was unlocked within 48hrs.
You may need to wait 30 days before they unlocked it; mine was locked for quite a while before I got the chance to unlock it.
If you need an LES in the meantime go to finance and tell them the situation
