Most “technical” failures aren’t caused by bad cryptography or broken protocols.
They come from mismatches between,
who thinks they own a system
who actually controls the keys, permissions, or upgrade path
and who’s on the hook when it breaks
The tech is usually fine.
The coordination isn’t.
Curious whether others here have seen the same pattern….especially as stacks get more layered and outsourced.
