You just need to pay per rental for it: https://www.americanexpress.com/en-us/insurance/premium-car-rental-protection/manage/

Just an FYI for when folks are using primary vs secondary coverage to differentiate cards they want to use. AFAIK, it's mainly the Visa Infinite (not signature) backed cards that always have it. And it's the main reason that I kept a card around. Now that I know that I can do that Amex Hack, other card is going.

Amex CAN offer primary rental coverage

Either way Amex is considerably cheaper than the rental counter. $19.99 or something like that

    2. Too bad Amex won’t approve me for the BCE when I have been approved by other issuers. Dunno if applying for the Amex Hilton cards would have better odds of approval.

It seems like this benefit can be used on any Amex card, but it is opt-in per card I believe. I’ve used it on my Delta Gold (and downgraded to Blue), but haven’t had to make any claims to test it.

Also the Expedia One Key mastercard (no AF) has primary rental coverage for some reason.

Chase Sapphire Preferred ($95 AF) would be the next cheapest with primary I believe.
