Me and my husband tried applying for Chase credit card in branch with banker.separate times and had banker call back office, all they say is they will mail decision. Perfect credit score and utilization plus we were pre approved. Anyone else this happened too? They keep saying they can’t verify some information.

This doesn’t provide any indication whatsoever whether you “should” have been approved or not. Your score in isolation is meaningless and can easily be gamed; lenders accordingly evaluate your overall credit *profile*. Utilization is also pretty unimportant for card approvals. Preapproval (in most cases, depending on *how* Chase presented it to you) is marketing, not underwriting.
But anyway, it’s not a particularly hard card to be approved for, so there’s something that’s wrong with your profile. Could be insufficient history, could be income, could be meaningless AU accounts that you think are giving you a “good” score but aren’t helping, could be velocity, could be overall limit with Chase already, could be a hundred other things.
The letter they mail you will tell you why you were denied, and will be far more useful than Reddit speculation.
Or maybe they couldn’t verify your info, like they seemingly already said.
