I have a car that is currently in my name and has been paid off years ago. I have a 17 year old niece that I'd like to gift my car to. Her guardian is saying I need to transfer the car title to them (the guardian) rather than to my niece. I just don't want to basically give my car to the guardian. I would like my niece to have a say in the car as it will be her car, not the guardian's. But they're saying insurance will be too high if I put it in my niece's name? Could the guardian not add the car to their insurance? Or can they only do that if their name is on the title? Will it change anything if I put both their names on the title? located in NC if that makes any difference

In most areas a minor cannot sign a legal contract until they are of legal age. So they cannot get insurance in their name and most areas you cannot get insurance for something you don’t own.
You could possible have your nieces name and her parent’s name on the title.
If you wait until she’s 18, problem solved. Can be in her name only. Her name will be on insurance policies whether her name on title or not since she’ll be primary driver.
