My adult son who does not live with us totaled his car. We have a spare car that is registered and insured that he can drive, do I need to add my son as a driver to the policy even though he doesn't live with us?

How long is he going to be borrowing this car? Like, a day or two while he gets a new car? Or is the intent that he’s just going to drive this car for the foreseeable future?

Yes and you need to update the garaging address to his address.

Yes, add him immediately and change the garaging for the vehicle to his address. If anything happens, the carrier would be within rights to either strip coverages to state limits or deny a claim entirely. He needs to be properly accounted for aka rated on the policy. If he totaled his car, does he have an insurance policy still or no? If not, even more reason he must be added to your policy. If he has no insurance of his own, your carrier would consider it premium evasion because he isn’t paying for insurance somewhere else.
