I moved some time ago and recently found out my VA license was never voided. It created more issues than I ever expected
To resolve things I’ve provided DMV with my State Farm policy details 3 separate times. Each time I pointed out that the required coverage details are provided. I pulled it directly from the app or sent over what my agent provided
Despite this they have rejected my submission because it does not have ‘Declaration’ page or statement listed in the header
The supervisor specifically told us ‘Declaration must be in the header’. Our agent has tried to help but is unable to modify the statements
It’s enraging and I don’t know what to do. I am at the point of switching carriers. I even called VA based SF agents to see if they could help. Nearly every poc has mentioned how difficult that department can be
Not sure if I should file a complaint with the state, bbb or SF corporate?
EDT: DMV is requesting our insurance information be formatted exactly in the way a few have posted. They even sent us a similar example. SF does not provide my insurance details in that exact format. They do not have Declaration Page stated at the top of the details page.
I’ve asked my agent and a few others, I sent the DMV provided example as well. I sent DMV the full renewal package to them more than once

DMV is likely looking for the official Renewal Certificate or Renewal Notice PDF rather than the general Policy Details screen. You can usually find this in the “Documents” section of the State Farm app; the first page of that specific file almost always has “Declarations Page” printed in the header. If that still doesn’t work, ask your agent for a Certificate of Insurance (COI) or call corporate to request a Certified Declarations Page. Don’t waste time with the BBB
I don’t work for State Farm and I haven’t had coverage with them for many years.
However, when I do a Google image search for “State Farm Auto Declaration Page”, I can see images of what State Farm dec pages for personal auto policies have looked like in recent years. (And some homeowners dec pages, and dec pages for other insurers… so be careful if you repeat the search.)
In the image results, I see a page that contains the words “DECLARATION PAGE” in large bold print, inside a box, at the upper right-hand corner of the page.
Now, it is possible that the document has been redesigned, or a different layout was approved/required in Virginia….but before you start filing complaints, it’s probably worth checking: does the document you’ve tried to provide look like what Google thinks the dec page looks like?
If “yes”, go to a DMV office and talk to a human, in person.
If “no”, ask your agent about the discrepancy.

Here is my coverage details page, I also have a mobile version. Neither has Declaration Page’ stated. This is what is available in my documents folder. My agent cannot modify it. They also consulted with their underwriter for a modification. They requested and provided a document from corporate which has my coverage details but doesn’t state ‘Declaration Page’.
I have asked several times for the inclusion. My agent did as well when requesting assistance from corporate
I am floored that your agent can’t provide you a declarations page?? It’s a specific document like the example image another user posted. It should be overtly obvious and easy to find.
